This patient developed basal cell cancer of the nose. She underwent Mohs excision by her dermatologist and Dr. Geiger reconstructed the defect with a “bilobed flap.” This relies on the plastic surgery principle of “replace like tissue with like tissue” whenever possible and used her remaining nasal skin to rotate over and close the hole. Although this adds extra incisions on the nose and can be scary at first, it heals amazingly well. Here she is seen during surgery with the cancer defect, the on-table closure, and again just 4 months after surgery. Note how the scars are hardly visible by that time.
